The report stated that a law firm employee in Essex, Laura Hayley Jackson, was banned from working in the legal industry after she left out the signatures of two clients on a Lasting Power of Attorney and decided to do them herself to save effort.

The LPA documents were then sent and registered at the Office of the Public Guardian. The two clients who are married then discovered their forged signatures on a receipt sent to them by the Solicitors Regulation Authority.

Jackson was later dismissed for misconduct by her workplace at North Ainley Solicitors in Oldham and fined £1,000 and ordered to pay £300 costs.
